Terms and Conditions

I hereby enroll in the 2015 EOI Performance and Adventure Camp and agree to the terms and conditions as outlined below. I agree to pay $4,500 total. $500 is enclosed as a non-refundable deposit and due upon registration. The second payment is due by February 1, 2015 and the balance is due by April 1, 2015.

The $500 deposit is non-refundable.  Additional money paid will be refunded in full on cancellations made prior to January 31, 2015, 50% refunded on cancellations made prior to March 31, 2015. Money is non-refundable with cancellations made after April1, 2015.

My signature indicates that there are inherent risks in off-ice training and there are risks of bodily injury from my participation in the activities offered by the EOI Performance and Adventure Camp and that I agree to release any claim I may have against, and, further agree to indemnify and to hold harmless in all respects, the camp, the camp owners, directors, staff, coaches, and other employees with regard to any claims for injuries or damages that may be incurred as a result of participation in said activities.

This application shall be construed and governed by the laws of the state of Texas. Any suit resulting from a dispute arising hereunder shall be pursued by either party only in the appropriate superior court in the county in Texas where the Excellence On Ice, LLC is located, or if jurisdiction will so permit, in the Federal District Court. The camp and the undersigned consent to venue and personal jurisdiction in such courts.
